Referral Information

Making a Referral to Pillarays Supported Living

At Pillarays, we’re committed to making the referral process clear, supportive, and responsive. We accept referrals from families, social workers, case managers, NHS professionals, and other care providers for individuals aged 18–65 with varying support needs.

Who We Support

Our Referral Criteria

We provide tailored supported living services for individuals who may have:
Learning disabilities
Autism spectrum conditions
Mental health needs
Complex needs requiring 24/7 support
Challenging behaviours
Acquired brain injury
Dual diagnosis (e.g., learning disability and mental health)
Transitional support (e.g., moving from residential care or hospital settings)

Adults aged 18 to 65
Require support to live independently
Eligible for supported living funding (local authority, NHS, or personal budget)
Willing to engage in a personalised support plan
May already have a tenancy or be looking for accommodation via our housing partners

Referral Process

Step 1: Initial Contact

Reach out to our referrals team by phone or email to discuss the potential placement. Contact Us:

Step 2: Submit Referral Form

Complete our Referral Form with details about the individual’s:
Current situation
Needs and preferences
Funding information
Risk assessments (if available)
Care or support plans (if applicable)
Use our Online Referral Form

Step 3: Assessment & Review

Our team will:
Review the referral within 48 hours
Arrange an initial conversation or visit
Conduct a needs assessment (virtual or face-to-face)
Liaise with care teams, families, and housing partners

Step 4: Placement & Transition Planning

If the referral is accepted:
We co-create a personalised transition plan
Assign a dedicated keyworker
Agree on start date and tenancy arrangements (if applicable)

Emergency or Complex Referrals

For urgent referrals or individuals with complex needs, please call our referrals team directly. We aim to respond rapidly and provide flexible support solutions where possible.
